Admissions (ADT)

ADT stands for Admission, Discharge, Transfer, the full HL7-standard lifecycle of a hospital stay. This module runs that lifecycle end to end: scheduling a future stay, admitting on arrival, moving between beds, and discharging.

01

What this module is for

Every inpatient stay follows the same standard events: Pre-Admit, Admit, Transfer, Discharge, or Cancel. This module records each one, keeps a live map of every bed, and flags stays that are running unusually long.

Who uses it

Admissions clerks and registration staff admit and pre-admit patients. Nurses, charge nurses and bed coordinators work the bed board. Case managers and social workers track discharge-ready patients and LOS outliers. Physicians and surgeons are recorded as attending/discharging providers.

Where to find it

Open Admissions (ADT) from the sidebar. Five screens: the admissions overview/list, pre-admissions, the bed board, a census report, and LOS outliers.

  1. 1

    Pre-Admit (optional)

A future stay is scheduled ahead of time, e.g. for a planned surgery, this reserves the slot but doesn't create a real stay yet.

  2. 2

    Admit

The patient arrives and is formally admitted, or a pre-admission is converted into a real admission.

  3. 3

    Transfer (as needed)

The patient moves bed, unit, level of care, or to another facility.

  4. 4

    Discharge or Cancel

The stay ends with a discharge disposition and summary, or is cancelled outright if it never should have started.

Admitting Order Notes

Admitted — currently in the hospital

Observation — being monitored, not a full admission

Pre-Admitted — scheduled, hasn't arrived yet

Transferred — moved bed, unit, or facility

Discharged — stay ended, patient went home or elsewhere

Cancelled — the admission was voided

03

Pre-admissions

Patients scheduled to be admitted in the future, e.g. for a planned procedure, reserved ahead of time but not yet checked in.

When the patient arrives, Convert to Admission turns the reservation into a real stay — confirm or fill in the Attending Physician (required if none is on file), Unit, Bed, Source of Admission, and Expected Discharge. Cancel voids a pending pre-admission and requires a typed reason for the audit trail.

04

Bed board

A real-time, visual map of which patients are in which beds right now.

Filter

FieldWhat it means
Filter by unit ID or nameNarrows the card grid to one unit.

Only currently-admitted patients are shown, one card per bed: patient name (with an isolation shield icon if precautions apply), MRN, bed, unit, attending provider, admission date, length of stay, ICD-10 code, and a status badge. Click any card to open that patient's full admission record.

05

Census report

How full one unit is on a given day, plus that day's admission, discharge, and transfer activity, for daily bed-capacity reporting.

An occupancy badge and progress bar at the top color-code capacity: green under 75%, amber 75–89%, red 90% and above. An "Active Patients" list below shows every current patient in the unit; click one to open their admission record.

06

LOS outliers

Patients whose stay has run much longer than expected, flagged for CMS/TJC utilization review and discharge-planning follow-up.

Threshold

FieldWhat it means
LOS Threshold slider3–60 days; quick-select buttons for 7d, 14d, 21d, 30d. Defaults to 14 days.

Each row shows a length-of-stay badge (amber up to 30 days, red beyond), patient, MRN, ICD-10 code, diagnosis, bed/unit, and attending provider. Click a row to open the admission record.

LOS = Length of Stay

An "outlier" is simply a patient who has been admitted longer than the chosen threshold, it's a worklist for case managers to review what's delaying discharge, not a clinical judgment on its own.
07

The admission record

Clicking any patient, from any of the five screens, opens the same detail drawer, with Discharge, Transfer, and Cancel actions and five tabs of history.

Overview

Admission type, patient class, source, last HL7 event, admitting diagnosis and ICD-10 code, attending provider, bed/unit, key dates, length of stay, and isolation details if applicable.

HL7 Events

A chronological audit trail of every HL7 ADT message generated for this stay, event code, description, timestamp, and who performed it.

Discharge Plan

Ongoing discharge-readiness tracking, separate from the final discharge: an Anticipated Discharge Date, Barrier Notes, and a checklist (medication reconciliation, teach-back, social work consulted, case management consulted). Save Discharge Plan.

Transfers

History of past transfers for this stay, type, reason, from/to bed and unit, and date.

An Interop tab exposes the raw FHIR R4 and HL7 A01/A03 data for this stay with a Copy button, technical detail meant for integration troubleshooting, not day-to-day use.

Cancel Admission requires a reason

Cancelling asks for a typed reason for the HIPAA audit trail before it will proceed, this is a permanent void, not a discharge.
08

Good practices

Convert pre-admissions, don't re-admit from scratch

Converting keeps the diagnosis, physician, and instructions already on file and links the two records together.

Check the bed board before requesting a bed

It reflects live occupancy across the whole unit, faster than paging through the census.

A valid NPI is required to admit or discharge

Attending and discharging physicians need a properly formatted 10-digit NPI on file, or the form will block submission.
