ICU / Critical Care

One critical-care stay's full record: admit with a severity score, chart ventilator and sedation readings, titrate vasopressors, set daily rounding goals, and discharge with a diagnosis.

01

What this module is for

Restricted to the staff who directly manage critical-care patients: physicians, nurses, nurse practitioners, physician assistants, respiratory therapists, and anesthesiologists, plus admins and directors.

Who uses it

Physicians and nurses admit and discharge patients and chart most readings. Respiratory Therapists record ventilator readings. Anesthesiologists and nurses record sedation and vasopressor data. Access is uniform across all three screens, no separate per-tab restriction.

Where to find it

Open ICU / Critical Care from the sidebar. Two screens: an Overview dashboard, and Admissions & Census, which opens into a 5-tab admission record.

  1. 1

    Admit

Pick a patient and ICU unit, optionally set bed, admission reason, and initial APACHE II / SOFA severity scores.

  2. 2

    Chart daily

Record ventilator readings, RASS/CAM-ICU sedation assessments, and vasopressor infusions as needed.

  3. 3

    Round

Set structured daily goals covering fluids, sedation, mobilization, weaning, and prophylaxis.

  4. 4

    Discharge

Close the stay with a discharge diagnosis once the patient is ready to leave the ICU.

A few terms you'll see throughout

APACHE II and SOFA are numeric severity-of-illness scores captured at admission. RASS (Richmond Agitation-Sedation Scale, −5 to +4) measures how sedated or agitated a patient is; CAM-ICU is a positive/negative delirium screen taken alongside it.
02

Dashboard

A read-only snapshot of census, severity, and device utilization across all ICU units.

Stat tiles

FieldWhat it means
Active CensusCurrently admitted ICU patients.
Avg APACHE II, Avg SOFAAveraged across active admissions.
On Ventilator, On VasopressorsPatients currently on each type of support.
Discharged (24h), Discharged (7d)

A "Census by Unit" bar chart breaks down patients by ICU unit. If there are no active admissions, the page simply shows "No active ICU admissions right now."

03

Admitting a patient

Search past and current admissions, filter to Active-only or by unit/patient, and admit a new one.

Admit Patient to ICU form

FieldWhat it means
Patient, ICU Unit*
Bed Number, Admission TimeAdmission Time defaults to now.
Admission Reason
APACHE II Score, SOFA ScoreOptional at admission, both can also be edited later on the Overview tab.

Submitting takes you straight to the new admission's record. The admissions list can be exported to Excel, respecting the active/unit/patient filters.

Active — ongoing ICU stay

Discharged — stay complete

04

Ventilator

Track mechanical ventilation readings over the course of the stay.

05

Sedation / RASS

Record sedation level, delirium screening, and sedative/analgesic dosing.

A running infusion shows an Active badge with a Stop button; stopping asks for confirmation and records the stop time.

07

Daily goals

A structured multidisciplinary rounding checklist, one entry per day.

08

Discharge

Close out an active stay.

The Discharge button appears only while a stay is active. Enter a Discharge Diagnosis and confirm — the admission flips to Discharged everywhere (list, detail badge, dashboard counts), discharge time is recorded, and the diagnosis becomes visible on the Overview tab. The admission record, including its full ventilator/sedation/vasopressor/goals history, can be printed or downloaded as a PDF at any time.

09

Good practices

Pair every RASS assessment with a CAM-ICU screen

They're recorded together on the same form, delirium screening loses its value if it isn't done at the same cadence as sedation checks.

Stop a vasopressor as soon as it's discontinued

An infusion left marked Active understates how many patients are truly on vasopressor support on the dashboard.

Set daily goals every day, not just on admission

The prophylaxis checklist (DVT, stress ulcer) and weaning plan are meant to be reviewed at each round, not filled in once.
